We would rather not, and we will tell you when a tooth can be saved and roughly what that would cost. When it genuinely cannot be, the appointment is straightforward — and we will have agreed what fills the gap before the tooth comes out.
A tooth is worth keeping when there is enough sound structure to restore, the root is not fractured vertically, and the gum support underneath is stable. Where those hold, root treatment and a crown will usually outlast the alternative even at a higher price.
Where they do not, delaying costs you bone. The ridge starts resorbing the day a tooth comes out and loses most of its width in the first year, which is exactly the bone an implant would need later. If an implant is likely, we can place a graft into the socket at the time of extraction and preserve it.
Surgical extractions — buried roots, curved roots, teeth that have broken at gum level — take longer and cost more, and we will tell you which yours is from the x-ray rather than after starting. Sedation is available at £280 if you would rather not be present for it.
The socket closes over in about three weeks and the bone fills in over three months. Expect two or three uncomfortable days and take the ibuprofen before the anaesthetic wears off rather than after.
Not always. A missing second molar at the very back often needs nothing. A gap further forward lets the neighbouring teeth tip and the opposing tooth over-erupt, and that is a slow problem that becomes expensive.
Intravenous sedation is available at both clinics with a trained sedationist present, at £280. You will need somebody to take you home and stay with you for the rest of the day.
